Commonly known as "Test Tube Baby". IVF means fertilization of an ovum outside the body and consequently transfer of the fertilized ovum (embryo) into the uterus of the women. IVF is probably the most widely practiced assisted conception procedure In the world. The procedure does not need admission at any step & is conducted on outpatient basis.

The procedure is indicated for irreparably damaged or blocked fallopian tubes and when other modes of treatments has failed in:

  1. Endometriosis.
  2. Unexplained infertility.
  3. IUI failures.
  4. Ovulatory dysfunction.
  5. Sperm disorders.
  6. Immunological problems.

Steps involved in IVF procedure

⇒IVF (Test Tube Baby), GIFT & ICSI, Ovarian stimulation by hormonal injection to produce multiple eggs.
⇒Monitoring of the response by ultrasound scans and blood tests.
⇒Egg retrieval with the help of a needle under local/general anaesthesia.
⇒The collected eggs and her husband's sperms which are mixed outside the woman's body in a culture dish or a test tube and are left for fertilisation in an incubator.
⇒Transfer of the resulting embryo(s) into the uterus of the woman on D2/D5.

Blood test performed 15 days after embryo transfer to assess the establishment of pregnancy. If the treatment procedure is successful, one or more embryos will implant in the uterus and the pregnancy will result, Just as it happens in the natural process of conception. The specific stages involved in IVF treatment will be explained in detail to each individual who comes in for treatment at 'HACC'.

Chances of success

Currently the success rate in our IVF Center is 35% - 40% for fresh early cleavage stage embryo transfer (D2/3) and 50% - 60% for blastocyst transfer (D5).

How does IVF work?
IVF techniques can differ from clinic to clinic, often depending on your individual circumstances.
A typical IVF treatment may involve:

For women:
Step 1. Suppressing the natural monthly hormone cycle
As a first step of the IVF process you may be given a drug to suppress your natural cycle.
Treatment is given either as a daily injection (which is normally self-administered unless you are not able to do this yourself) or a nasal spray. This continues for about two weeks.

Step 2. Boosting the egg supply
After the natural cycle is suppressed you are given a fertility hormone called FSH (or Follicle Stimulating Hormone). This is usually taken as a daily injection for around 12 days.
This hormone will increase the number of eggs you produce - meaning that more eggs can be fertilised. With more fertilised eggs, the clinic has a greater choice of embryos to use in your treatment.

Step 3. Checking on progress
Throughout the drug treatment, the clinic will monitor your progress. This is done by vaginal ultrasound scans and, possibly, blood tests.
34–38 hours before your eggs are due to be collected you have a hormone injection to help your eggs mature.

Step 4. Collecting the eggs
In the IVF process eggs are usually collected by ultrasound guidance under sedation. This involves a needle being inserted into the scanning probe and into each ovary.
The eggs are, in turn, collected through the needle.
Cramping and a small amount of vaginal bleeding can occur after the procedure.

Step 5. Fertilising the eggs
Your eggs are mixed with your partner’s or the donor’s sperm and cultured in the laboratory for 16–20 hours. They are then checked to see if any have fertilised.



Those that have been fertilised (now called embryos) are grown in the laboratory incubator for another one - two days before being checked again. The best one or two embryos will then be chosen for transfer.

After egg collection, you are given medication to help prepare the lining of the womb for embryo transfer. This is given as peccaries, injection or gel.

Step 6. Embryo transfer
For women under the age of 40, one or two embryos can be transferred. If you are 40, or over, a maximum of three can be used.
The number of embryos is restricted because of the risks associated with multiple births. Remaining embryos may be frozen for future IVF attempts, if they are suitable.

For men:
Step 1. Collecting sperm
Around the time your partner’s eggs are collected, you are asked to produce a fresh sample of sperm.
This is stored for a short time before the sperm are washed and spun at a high speed. This is so the healthiest and most active sperm can be selected.
If you are using donated sperm, it is removed from frozen storage, thawed and prepared in the same way.

IVF Failure

How to deal with IVF Failure?

The feeling of breathing and growing someone inside you, is just amazing and no one can describe this feeling in words for failed IVF treatment. Sometimes due to unexplained reasons, the embryos transplant during IVF treatment, consistently fail to implant and results in IVF failure.

IVF is not a miracle solution for infertility, instead it’s true that it is the most successful treatment when all other ART techniques have failed. One of the reasons for IVF failure is the abnormality of embryos or they may be chromosomally abnormal, due to which they fail to implant.

Failed IVF cycle is a heart wrenching condition as your dreams are shattered and your money also goes down. You feel like everything is finished and some also start thinking that they can never become parents and God will not allow them to enjoy parenthood. Actually infertility specialists are also not able to give the answer after failure, but as a human being, we are curious to know why did my cycle fail? And what are the other chances to conceive again? Etc.

Giving the exact reason for IVF failure is not always feasible to describe to the patient as still there is no sure way to find out which embryo is genetically normal and which is not. Patients are not comfortable with the fact that their doctor is not aware about the real cause of IVF failure and they start doubting on the doctor efforts and experience. In this condition, you need to understand that doctor is not a God and the procedure is not a miracle, every treatment has its own risks involved and there is no valid explanation for an IVF failure.

In case, IVF fails, stand up and face it all, don’t think that so much crying and suicidal will help you for this. Come on women, you are strong enough, if you are able to give birth to new life, you have enough strength to face it all, if (in case) it fails.

You can go for second IVF cycle, advisable after one week and if you feel scary in second chance, can try for natural remedies and take a leap of faith, as there are various ways available to treat infertility. Put together your all hopes and try to hope for better.

Treatment Options Available For Infertile Couples

  • Fertility drugs: Fertility medications are available that helps to achieve pregnancy like clomiphene pills, gonadotropin injections, etc. which triggers the egg production. Ensure to consult with your doctor about the fertility drugs as some of them has side effects also and the pregnancy rate is about 28% with the help of these fertility drugs or medications. These are especially recommended for women who do not ovulate regularly and the men who have poor quality sperm. But in case of blocked or damaged fallopian tubes, avoid it.
  • IVF (In Vitro Fertilization): It is one of the most successful ART techniques and mostly recommends when all other options have failed. It is the technique in which doctor removes eggs from your ovaries, fertilized them with the sperm in laboratory. Once embryos are ready, they are transplanted back into your uterus. Success rate of IVF varies from age to age, means in older age success rate is low but with the help of this technique, couples with serious infertility issue can become parent. Other treatment components of IVF are ICSI (Intra- cytoplasmic sperm injection), IUI (Intrauterine insemination), sperm donation, egg/embryo donation etc.
  • Surrogacy: Surrogacy is the third party arrangement procedure in which a woman carries or gives birth for another woman and in return she gets compensation. The woman, rent her womb is known as surrogate mother, who became pregnant by artificial insemination with the male partner’s sperm or by using donor sperm through IVF procedure. In case where both partners are infertile or where female is unable to conceive even with the artificial techniques, surrogacy is best option.
  •  Adoption: Adoption is the best way if you want to help those children who do not have parents and waiting for good life. The adopted child is not genetically related to the intended parents and it also requires so much legal documentation procedure, instead it is good if you do not want to suffer pain and want to avoid hospital travelling.

Male Infertility

Male factor is at least partly responsible in about 50% of infertile couples and may be the sole reason for couples who are unable to conceive after one year of sexual intercourse. Studies reveal that one out of every five couple, suffering the trauma of infertility and not because of only female health issues, as both are equally responsible for the conception.

Various reasons are there which give rise to male infertility such as smoking, alcohol intake, exercising, exposures to environmental hazards, inadequate diet, wearing tight under wear, excessive stress etc. The main reason for male infertility is the abnormalities of the sperm like low sperm count, poor sperm movement, sperm absence and hormonal deficiencies etc.

Male infertility is as complex as female infertility, it is advisable to go for fertility test, so that with the help of physical examination, doctor will check the quality and quantity of sperm, which is an important part for the semen analysis and can detect the problems with sperm motility, sperm morphology, azoospermia and oligospermia etc.

Depending on the result of physical examination, it is easy to determine the type of male infertility such as:

  • The man who have blockage in the genital tract that prevents the passage of sperm between the testes to the penis, which results in the absence of sperm, known as azoospermia. In this condition, sperm is obtain by a surgical operation from the tubes and can be used in the ICSI and IVF treatment.
  • Due to the hormones deficiency from the pituitary gland which mainly controls the testicular function, sperm motility affects which affects less than 1% of infertile men. This can be treated with the help of some injections of hormones preparations.
  • Some genetic causes are also there for male infertility which requires investigation and with the proper counselling it can be treated as well.
  • Other condition may cause poor semen quality but facts has not been proved that treatments improve the semen quality, instead by avoiding smoking, alcohol and other drugs which affects the sperm quality, anxiety, mental stress etc. is the only way to improve semen quality.
